Please fill in the student's name, circle the correct program, and fill in the homeroom teacher if you know their name. Next, sign and date the bottom and return the form to your student's teacher or front office. Devices are sent home after the beginning of the school year. It is to be kept at home in a safe location during the school year with the intent it will be used for Remote Learning or E-Learning Days. The device is for educational purposes only! The device will be returned at the end of the school year or if your student transfers out prior to the end of the year, whichever date is first.
 
ABLE and EC-MacArthur use the form "Equipment Checkout Agreement Form ABLE and MacArthur.pdf." All other programs use the form "Equipment Checkout Agreement Form Chromebooks.pdf."
