Physicals - Kindergarten, 6th and 9th Grade

 

 

 

Physicals are required for students entering kindergarten, 6th grade and 9th grade. Please take a copy of the State of Illinois Certificate of Child Health Examination below to your child's physical exam appointment. Return to your program's office.
